Mouth-to-mouth not necessary for effective CPR

NEW YORK (Reuters Health) - A simpler version of cardiopulmonary resuscitation (CPR) administered outside the hospital by bystanders - one that uses chest compressions without mouth-to-mouth resuscitation -- may be just as effective as standard CPR, which includes mouth-to-mouth resuscitation and chest compressions.

That's according to two studies published online on December 10 in Circulation: Journal of the American Heart Association.

Dr. Leif Svensson, from Karolinska Institute in Stockholm, and colleagues compared the two methods of resuscitation among people who suffered an out-of-hospital cardiac arrest, received any form of bystander CPR, and were reported to the Swedish Cardiac Arrest Register between 1990 and 2005.

Their cohort included 11,275 patients: 73 percent received standard CPR, 10 percent received compression-only CPR, and 17 percent received only mouth-to-mouth CPR.

Twenty percent of the patients treated with compressions only and 19.6 percent of patients who received standard CPR survived until their arrival at the hospital. After 1 month, 6.7 percent and 7.2 percent, respectively, were still alive, a nonsignificant difference. The outcomes were worse for those given only mouth-to-mouth resuscitation, with only 4.5 percent alive at 1 month.

The researchers note that early initiation of CPR improved outcomes. However, "studies have shown that many rescuers fail to respond or respond insufficiently, which in some cases is due to fear of contagion or repugnance," Svensson notes in a journal press release.

"If you hesitate to do CPR, remember that chest compressions only is better than doing nothing," added Katarina Bohm, a registered nurse who was involved in the study.

Dr. Taku Iwami, at Kyoto University Health Service, and associates found similar results when they analyzed more than 4,900 witnessed cardiac arrests. Specifically, they found higher 1-year survival with favorable neurologic outcome when bystanders provided either compression-only CPR or standard CPR compared to no CPR when emergency medical personnel arrived within 15 minutes to provide additional care.

Iwami also comments in the press release that the low rate of bystander CPR may stem from the fact that "conventional CPR requires that bystanders perform a complicated task for an emotionally intense situation."

"Cardiac-only (chest compression-CPR) is a much simpler technique that is easier to teach, learn, remember, and perform," he adds.
