The age adjusted incidence of cancer of the oropharynx observed in males in Colorado increased 36.6 percent when comparing the decade 1980-1990 with the decade 1991-2001. This was associated with a change in the observed ratio of human papilloma virus (HPV)-positive to HPV-negative cases from 0.72 prior to 1995 to 3.81 afterward. Disease specific survival was superior in HPV-positive patients. This data suggest HPV status may influence oncogenesis. It also suggests HPV status should be considered in choice of treatment.

Deferring Planned Neck Dissection Following Chemoradiation for Stage IV Head and Neck Cancer: The Utility of PET-CT.

A cohort of patients with advanced (N2 or N3) cervical metastasis from squamous cell carcinoma were treated with chemoradiation therapy. Monitoring was undertaken with positron emission tomography and computed tomography (PET-CT) in 43 patients. PET-CT suggested persistent disease in ten patients of whom three (30%) were false positives. The positive predictive value was 70%. In the cohort of patients with negative PET-CT, the negative predictive value was 97%. This suggests that most patients can be safely followed without surgery in the setting of a negative posttreatment PET-CT scan.

Value of Sestamibi Scans in Tertiary Hyperparathyroidism

Tertiary hyperparathyroidism (THPT) results from proliferation of parathyroid tissue, in the setting of hypersecretion of parathyroid hormone in patients with long-standing chronic renal failure. This generally results in four-gland hyperplasia. In a retrospective review by Dr. Loftus and colleagues of 21 patients with THPT, ectopic glands were identified in 43% of patients. The authors argue that the preoperative localization with sestamibi is valuable even in the face of four-gland hyperplasia in as much as it may allow localization of ectopic thyroid tissue and contribute to surgical success.
