Super Selective Embolization As First Step Of Laparoscopic Partial Nephrectomy

 
UroToday.com- While laparoscopic radical nephrectomy has been widely touted as the new standard of care in the treatment of localized renal cell carcinoma (RCC) laparoscopic partial nephrectomy (LPN) has failed to gain such widespread acceptance due to the steep learning curve and technical difficulties associated with the successful application of this approach. Prolonged ischemia times due to the technical challenges of intracorporeal suturing and difficulties with the application of hemostatic agents to augment hemostasis remain technical challenges that are difficult to overcome with current technology. Here, Gallucci and colleagues report on the use of super selective preoperative embolization of the vascular supply to a renal tumor prior to LPN as a hemostatic adjunct.
The authors report on 50 patients who underwent angiographic super selective embolization 6 hours prior to LPN. Mean OR time was 90 minutes (mean embolization time was 43 minutes) and mean blood loss was 200 ml. Mean length of stay was 6 days and mean tumor size was 3.5 cm. RCC was diagnosed in 43 cases and with a median follow-up of 11 months, no recurrences or positive margins were noted. Of note, after embolization, hilar vascular control and clamping is not utilized. The authors report two complications, an infected hematoma that required drainage and a patient that actually had a hilar mass not deemed appropriate for LPN intraoperatively and therefore converted to laparoscopic nephrectomy.

The authors report that super selective embolization is an effective adjunct to hemostasis in LPN that avoids the potential complications associated with prolonged renal ischemia that can be problematic in conventional laparoscopic approaches associated with hilar clamping. Further experience with this technique will determine whether it truly is a viable option that is both cost-effective and avoids the unnecessary loss of normal renal parenchymal tissue due to inadvertent loss of vascular supply in the embolization process.
