Microvascular Tumor Invasion: Prognostic Significance In Low-Stage Renal Cell Carcinoma
UroToday.com- The importance of microvascular invasion (MVI) of renal cell carcinoma (RCC) as a prognostic factor has been much debated in the literature of urology. Opinions range from no significance to the most significant histologic variable that can be reported. Here, Madbouly and colleagues report their outcomes for patients with RCC examining MVI as a prognostic variable.

The authors report on 8 patients with localized RCC with at least one year of follow-up who underwent radical nephrectomy over a 14 year period. Of these, 6 developed metastatic disease in follow-up and ultimately died of RCC. Eight patients were noted to have MVI of these, 50% developed metastatic disease whereas only 5% of patients without MVI developed metastatic disease. The presence of MVI was associated with male sex (p=0.017) and increased stage (p=0.039). The 5 year disease free survival was only 45% for those with MVI versus 90% for those without. In their multivariate analysis that included stage, grade, histology, tumor size, age, the presence of perinephric fat invasion, and MVI only MVI was a significant predictor of disease free survival (p=0.007).

The authors conclude that MVI is a significant predictor of patient outcome that should be reported on all patients undergoing surgery for RCC.

The small numbers of the study and the fact that patient sex was also significant in predicting outcome make these conclusions somewhat suspect but the findings regarding MVI concur with many other larger studies in the literature.
